I give permission for camp nurse to administer my child’s prescribed medications according to instructions provided.

I give permission for camp staff to administer over-the-counter medications or provide first aid as needed (e.g., bandages, antiseptic, pain relief).

Cabin buddy request (up to 2 names). We’ll do our best to accommodate requests, but cannot guarantee placement.

I authorize Mountainside Bible Camp staff to seek medical attention for my child in the event of an emergency.

I understand that participation in camp activities involves some risk, and I release Mountainside Bible Camp from liability for injuries incurred while at camp, except in cases of gross negligence.

I give permission for photos or videos of my child to be used for camp promotional purposes (website, social media, brochures).

Pay $100 now to reserve your camper’s spot. The remaining $200 is due at drop-off.
